Before filling out the required information, please read the following document thoroughly. All parents/guardians and students MUST read, understand, and accept the terms of the Production Packet to participate in a Liberty High School theatre production.

Liberty High School Theatre Participation Acknowledgement Form
We, the Parent/Guardian and Student, acknowledge that we have received the Production Packet regarding the Liberty Lions’ Pride Players production of Eurydice. We have read and understood the entire packet and recognize that failure to comply with any rules within the packet may result in removal from the production.
Production Fee: As a member of the Cinderella cast and/or crew, each student is required to pay a $60 production fee. This fee helps cover costs for costumes, props, sets, lighting, sound, special effects, and other aspects of our productions. Please note that the production fee is non-refundable.
This submission will serve as your virtual signature, indicating that you have read, understand, and agree to the terms outlined in the Production Packet.
